were removed from memory. The volumes are still intact on disk.
Because each volume on a given server must have a unique name, take one of
the physical disks offline. You might have to delete one of the volumes and
then bring the disks online simultaneously.

A volume segment was discovered that is either corrupted or is only a part of
a complete volume.
Make sure that all segments of the volume are online and drivers are
