Two volumes with the same name exist, but their block sizes differ.
Bring the disks that contain these volumes online one at a time, rather than at
the same time. If one of the volumes is old and no longer needed, delete it to
avoid the problem in the future. Two volumes on the same server cannot have
the same name.

This error is probably caused by insufficient memory.
