Meaning
Microcode download failure
The adapter microcode download
fails.
Adapter card test failure
The diagnostics detect a failure during
initialization of the adapter.
Open phase test failure
Action
Make sure the adapter is firmly seated
in the adapter slot.
Make sure the adapter microcode is
loaded to the correct disk or diskette.
Run the diagnostics again. If the
failure continues to occur, follow the
remove and replace procedures for
the TOPS MPX position. This
procedure is located in the common
procedures section of this manual.
Make sure the adapter is firmly seated

If necessary run the automatic
configuration procedure.
Run the diagnostics again. If the
failure continues to occur, follow the
remove and replace procedures for

Replace the adapter cable with a
spare working cable until the problem
is fixed.
Run the diagnostics again. If the

adapter cable is defective. Replace
the damaged cable.
