Any time service is performed on the LTV 1200, 1150, or 1100 ventilator, a Service
Record form should be filled out and returned to CareFusion. This enables complete
tracking of replacement part lots and allows CareFusion to maintain comprehensive
service history records.
A Service Record form should be filled out for all types of service, including part
replacement and calibration. An example of a blank Service Record form can be found in
this appendix. Contact CareFusion to obtain a blank Service Rercord form to complete
