index marks the most recent call record.

When this setting is turned on, pressing the CANCEL-key will cause
call-termination with all parties in conference.
When this setting is turned off all parties will be held instead. HOLD-key
always holds all conference members.
For onhook/offhook it can be combined with setting "conf_hangup".
