as <chassis label> has reported that the consistency initialization pass has
completed for logical drive <logical drive number>.
The external Smart Array controller is attached to the host side Smart Array
controller <controller name / slot information>.

Log message: The external Smart Array controller located in the MSA chassis labeled
as <chassis label> has reported an uncorrectable read error during surface
analysis operations for logical drive <logical drive number>. A media error was
encountered that is not correctable due to media errors on other physical drive(s)
belonging to this logical volume. The uncorrectable media defects are between
logical block address <lba number 1> and logical block address <lba number 2>.
The host will be unable to read some blocks between this address range until the
blocks are overwritten. Capacity expansion operations must be avoided while the
blocks are unreadable.
The external Smart Array controller is attached to the host side Smart Array
controller <controller name / slot information>.
