Completion Instructions

The field engineer performing the installation must complete this form on-site.
Place N/A in all spaces that are not applicable to this customer site or installation. Do Not Leave
Blank Spaces.
The field engineer must complete this form as soon as all hardware is installed and tested. Any and
all exceptions must be noted where specified.
When the form is completed, you must forward it to your immediate supervisor for distribution.

Installation Requirements

Completion of the following items constitutes field operation requirements for hardware installation.
1. The shipper places the system in its permanent location and removes the shipping material.
The system must not be moved by installation personnel.
2. An inventory of the order is completed.
3. All cables brought into the system are connected internally to the cabinet. All intercabinet
cabling that came with the order is connected to each cabinet.
4. The system is setup in accordance with the Avaya MPS 500 commissioning guide.
5. The system is powered up, all internal self-tests run successfully and proper bootup is
observed.
6. The Field Factory Test is run to test that all installed products are fully operational.
7. Any approved diagnostic is run on all application processors installed to assure they are fully
operational.
8. The system can be networked internally to other Avaya products and to the customer LAN
according to the LAN diagram provided by the customer.
9. All licensing must be verified to match the order.
