1. On page 1, fill in the customer name, telephone number, and address.
NOTE:
Reference Number is filled in by NTM.
2. Fill in the Order Number.
3. Identify the personnel handling the account:
a. Account Executive's name and telephone number.
b. System Consultant's name and telephone number.
4. Identify the NI-1 BRI facility vendor by writing the vendor's name under the
NI-1 BRI Vendor heading, along with the contact person's name and tele-
phone number.
5. Under the Installation heading:
a. Write the "Due Date" on which the installation is scheduled to be com-
