This section provides overview information on the basic operation of the BD FocalPoint™ GS Imaging System and a brief review of the workflow process for orientation. Specific details on how to operate the BD FocalPoint™ Slide Profiler and BD FocalPoint™ GS Review Station are discussed in subsequent sections.
BD FocalPoint™ GS Imaging System Instrument User’s Manual BD FocalPoint™ GS Review Station The BD FocalPoint™ GS Review Station is designed for the desktop and allows the user to perform a computer assisted slide review. It consists of a Main Window (monitor), specialized guided screening software and an automated review microscope.

Table 3-1 – Menu options and tasks When the BD FocalPoint™ GS Review application is first opened, the user is instructed to calibrate the stage. Calibrating the Stage In order to set the x and y coordinates of the microscope stage, a stage calibration must be performed to establish the reference point.

Guided Screening Use the following steps to calibrate the stage. NOTE Perform the calibration process with the same magnification (for example 10x) that is used for screening slides. Put the slide securely on the stage with the label on the left. WARNING MAKE SURE THAT THE CLIP GRIPS THE SLIDE EDGE SECURELY.

Guided Screening NOTE To aid in locating the right side edge, bring the slide to the top right corner so that the top edge and right edge are in view. Then center the right edge. This aids in visualizing the actual edge of the slide as the clip may appear too dark to allow for slide vs.
Position the cursor in the Enter Slide ID box, and delete any numbers already displayed. Use the BD FocalPoint™ GS Review Station keyboard to type in the number from the slide label. The BD FocalPoint™ GS Review Station will recognize the number and access the information in the database.
To identify the slide by selecting it from the slide list, use your mouse to select the slide accession number from the list. The BD FocalPoint™ GS Review Station will access the information in the database. Your slide is listed at the top of the slide list and the processing results are displayed in the slide results data area.

These black and white, 20x magnification images are not to be used for diagnostic purposes and are provided for location verification only.
BD FocalPoint™ GS Imaging System Instrument User’s Manual 3.4.2 Verifying the Stage Position/Using the Offset Function When reviewing FOVs, the stage moves according to the location coordinates recorded in the database for that slide. Before beginning reviewing, the stage position must be verified.
Navigating Through FOVs Remember, the Review button enables FOV review. If the slide has been successfully processed on the BD FocalPoint™ Slide Profiler and FOV images are available, the system enables the review mode. Use the mouse, foot pedal or Navigation buttons to navigate to each FOV as described below.

Samples of low or inadequate squamous cellularity must be considered at increased risk for harboring a significant lesion, and thus deserve additional attention during the screening process. Awaiting further review (AFR) is selected when the slide requires further review by a second individual such as a pathologist. Slides queued up as Awaiting further review can be retrieved using the BD FocalPoint™ GS Review Station in either GS review mode or by the second reviewer.

Manual full slide review (FSR) Manual Screening A full slide, manual review is performed at the BD FocalPoint™ GS Review Station using the microscope’s manual stage controls. Areas of interest on the slide can be marked using traditional methods such as a dotting pen.

Information on how and when to use these features is described as it occurs in the workflow (refer to Section 5 for additional information).
Screening Slides When adding an electronic dot, the BD FocalPoint™ GS Review Station adds a blue dot to the slide map and saves the location coordinates. Figure 4-3 illustrates the BD FocalPoint™ GS Review Station display as it appears when adding a new dot.

BD FocalPoint™ GS Review Station with this interface. The LIS button is turned off when the BD FocalPoint™ GS Review Station is first started. Before this button can be used, settings to identify your LIS application must be customized by the BD Service personnel.
BD FocalPoint™ GS Imaging System Instrument User’s Manual The Offset button will re-align the x/y coordinates of the stage position as needed after calibrating the stage. Refer to Verifying the Stage Position/Using the Offset Function in Section 3 for details on how to perform the offset.
The Help button provides a help file and an index of useful help topics. The Help window opens in a secondary window that can be re-sized and positioned as desired. Either keep it open for future reference, or close it to return to the BD FocalPoint™ GS software application. 5.1.5 Most Significant Dot Button Figure 5-6 illustrates the use of the Most Significant Dot function.

The Navigation buttons are: Beginning (back to the first dot), Rewind (back one dot), Forward (ahead one dot), and End (ahead to the last marked dot). LOIs marked by the BD FocalPoint™ Slide Profiler appear as black dots in the slide map area.

BD FocalPoint™ GS Imaging System Instrument User’s Manual 5.3.1 Slide Status Information Slides are assigned different icons with associated colors based on their processing and interpretation status. As illustrated in Figure 5-14, these icons are displayed just to the left of the slide list.
Profiler. Usually, this is because the slide was indicated as Rerun or Process Review the first time through the BD FocalPoint™ Slide Profiler and was put back into the system. If a slide with an M next to it is selected for review, a dialog window similar to the one in Figure 5-16 will appear.

Table 6-1 – Operating modes 6.1 Enhanced QC Review option The enhanced QC option notifies the user of slides that the BD FocalPoint™ Slide Profiler designated as QC Review. This option is enabled by default. This notification (see Figure 6-1) occurs when the slide is assigned the Ready for Signout status.

Managing the BD FocalPoint™ GS Review Station Server This section discusses the flow of data into and out of the BD FocalPoint™ GS Server. Two utilities manage this process: Copy Service and Archive. • The Copy Service transfers data to the BD FocalPoint™ GS Server.
Double-click the FocalPoint™ GS Copy Service shortcut on the desktop of the BD FocalPoint™ GS Server. Slide data for all print sets run on the BD FocalPoint™ Slide Profiler, but not yet downloaded, is copied to the BD FocalPoint™ GS Server. Once copied, slide data is ready to review.
No slide information or images would have been sent to the workstations. Call BD Technical Service and Support, and a representative can walk the system administrator through the steps.
Under normal use the server accumulates significant amounts of slide data. For example, full capacity use of the BD FocalPoint™ GS Review Station could generate as much as 450 MB of data a week (7 business days) per each BD FocalPoint™ GS Review Station.
7.3.1 Signing Out a Batch of Slides The BD FocalPoint™ GS Review Station is a closed system (unless it has been interfaced with your LIS system). Signing out a case refers to removing it from the slide list at the BD FocalPoint™...

In the Date Selection area, use the calendar to select the day for sign out. Slides processed on the BD FocalPoint™ Slide Profiler before the selected date will be signed out. For example, if May 17 is selected, slides processed through May 16 will be signed out.
Slides that meet your selected criteria will be unsigned out. NOTE Archived slides cannot be unsigned out. Archive After clicking Sign Out on the Slide Sign Out window, the Archive button turns green and the Archive Signed Out Slides window appears for archiving previously signed out slides.

Click Archive Now. The slides that have met the selected criteria will be archived. These slides will be moved out of the database and will no longer be part of the slide list on the BD FocalPoint™ GS Review Station.

NOTE The name of the folder starts with “SWArchive” and also contains the date and time that slides were archived. Click on the + for the desired archive folder. Viewing Archived Slide Data Once the slides have been archived (removed from the database on the server), they might need to viewed again. This can be done by opening the Archive Application at the BD FocalPoint™ GS Review Station. First, ensure that the BD FocalPoint™ GS Review Station has been properly mapped on the network to receive this information.

Generating BD FocalPoint™ GS Review Station Reports This section discusses the reports available from the BD FocalPoint™ GS Review Station. To view, customize, or print slide data reports, use the BD FocalPoint™ GS Review Station Report utility. BD FocalPoint™ GS Imaging System Instrument User’s Manual 8.1.2 Selecting the report type Select the type of report to be created. From the Report utility work flow panel, click the Select Report Type button. The Select Report Type panel is displayed beneath the work flow panel.
There are three types of reports that can be generated: summary, custom, and QC. 8.2.1 Summary Reports Summary reports can either display a summary of BD FocalPoint™ GS Review Station processing activity for the selected time period (month, week, or day), or all of the slides currently stored in the database.

8.2.2 QC Reports The QC report details the progress of BD FocalPoint™ GS Review Station processing for each slide in the print set. Generating interim versions of this report and analyzing this progress determines when it is appropriate to generate the Final QC Report that details which slides are the most appropriate candidates for QC rescreening.
8.2.3 Generating the Final QC report As described previously in this section, the BD FocalPoint™ GS System Results - By Rank report details the progress of BD FocalPoint™ GS Review Station processing for each slide in the print set. When generating a QC report, if all slides in the print set have not yet been reviewed, the report is considered preliminary.

The BD FocalPoint™ Slide Profiler Tray Handling System (not pictured) The tray handling system moves trays from the input hopper to the microscope stage, to various scanning positions on the stage, and to the output hopper. Trays are lowered with an input hopper elevator, pulled onto the stage with a transport chain, positioned on the stage, and when processing is complete, raised to the output hopper.
Each prepared Pap test (BD SurePath™ or conventional Pap smear) slide is affixed with a slide barcode label and loaded into a BD FocalPoint™ slide tray, which holds up to eight slides. Up to 36 trays can be placed in the input hopper for automatic slide processing.
QC Review (Rescreen) Slides During normal operation, up to 15% of the highest ranking slides interpreted as NILM by the cytotechnologist and classified as Review by the BD FocalPoint™ Slide Profiler are selected for directed QC rescreening. Refer to Section 19 for a detailed description of recommended QC procedures.
Slides receiving an action of Process Review could not be analyzed because of slide physical characteristics or specimen limitations. Some Process Review slides with physical damage can be repaired and rerun on the BD FocalPoint™ Slide Profiler. If a slide cannot be rerun, it must be manually screened.
10.4.1 Ranking Information Some of your Review slides may have been classified by the BD FocalPoint™ as scant cellularity. These slides do not have associated ranking information. The reason for the scant cellularity classification is that there are not an adequate number of reference cells on the slide to successfully qualify and rank the slide.
Processing Status Information Each slide result report confirms if the slide was completely and successfully imaged by the BD FocalPoint™ Slide Profiler. If a slide was not successfully processed, a message stating the reason for the unsuccessful scan accompanies the status.
The BD FocalPoint™ Slide Profiler Main Window The BD FocalPoint™ Slide Profiler consists of an instrument and a computer workstation. The instrument processes the slides and calculates the slide results. The workstation, consisting of a computer and a color monitor, runs the workstation software that displays the windows-based, graphical Main Window.
11.1 Main Window The BD FocalPoint™ Slide Profiler Main Window is the highest level window. That is, it appears when the workstation software is started and it remains visible while performing various screening tasks. Figure 11-1 illustrates a typical BD FocalPoint™ Slide Profiler Main Window.
A Home button to the right of the instrument bar indicates which instrument screen is in use and can be used to return to the Main Window. See Section 15, Main Window Operating the BD FocalPoint™ Slide Profiler, for detailed information on the Main Window instrument icons.

Operating the BD FocalPoint™ Slide Profiler 15.1 General Warnings and Cautions Observe the following general warnings and cautions while operating the BD FocalPoint™ Slide Profiler. Other, more specific warnings and cautions appear throughout this section and elsewhere in the manual.
15.2 Preparing Slides for Processing For slides to be successfully processed on the BD FocalPoint™ Slide Profiler, they must meet specific physical standards as described in Section 13. Additionally, each slide must also undergo certain cleaning procedures and have a properly applied BD-approved barcode label.
To run both coverslip type slides on a BD FocalPoint™ instrument for the first time, requires a service call. Both coverslip types cannot be run on the same BD FocalPoint™ instrument until the instrument has been properly configured.
15.3.2 Changing Preparation Types with Switch Slides If processing more than one type of slide preparation on a single BD FocalPoint™ Slide Profiler, a special slide must be inserted in the tray to separate each type of preparation. These special slides are called switch slides.
A stack of trays can be placed in an idle BD FocalPoint™ Slide Profiler and processing initiated, or additional trays can be added to the input hopper during processing without interrupting the processing sequence.
BD FocalPoint™ GS Imaging System Instrument User’s Manual After the first tray is inserted properly, several slide trays can be stacked and added to the hopper as a unit (a total of 36 trays). Close the hopper door securely to begin processing. Note that the instrument will not start processing until the hopper door is properly closed.
Operating the BD FocalPoint™ Slide Profiler 15.5 Monitoring the Instrument during Processing After loading trays in the input hopper and closing the door, the system automatically begins its calibration sequence, which takes about ten minutes. After the system completes the calibration, the first tray automatically advances under the microscope objective and slide processing begins.

To protect your data, BD recommends establishing a routine backup procedure. The slide processing data is stored on the hard disk of the BD FocalPoint™ Slide Profiler, and each day at approximately 1:00 AM the system automatically copies important system and slide results data from the hard disk to a removable storage device.
15.11.2 Backup Media Archive Reminder Slide data is stored on the BD FocalPoint™ Slide Profiler for a maximum of 2 months. Billing and System Integrity information will be stored on the device for a minimum of 13 months, and a maximum of 14 months.
The Shutdown command closes down all functions of the BD FocalPoint™ Slide Profiler computer. Occasionally, a BD-authorized technical or field support employee may ask to do this for troubleshooting purposes. This may also be needed if the BD FocalPoint™ Slide Profiler needs to be shut down for an extended period.

BD FocalPoint™ Slide Profiler Processing Messages When slides are rejected because the scanning process did not complete, or because the slide or specimen was unsuitable, a reason is printed on the report. Refer to this section for a description of these reasons. In some cases the slide may be repaired and rerun. If the laboratory identifies a subset of slides that cannot be rerun through the device, these slides must be screened by a cytotechnologist.

Occasionally, this error can occur when the current slide does in fact match the expected preparation type. For example, this situation might occur if a BD SurePath™ slide had an excessive number of cells outside the 12 mm specimen circle.
21.2 Scant Cellularity The BD FocalPoint™ Slide Profiler evaluates each slide to verify that it contains enough cellular material to be processed. If any one of the cellularity limit checks fails, the Processing Result will be Scant cellularity with description of the failure given.
21.3 Staining Not Within Limits The BD FocalPoint™ Slide Profiler evaluates each slide to verify that it is suitably stained. Stain characteristics of the slide are analyzed with respect to their influence on the scanning performance of the instrument. A slide determined by the system to be unsuitable for analysis is reported as Staining not within limits with the description of the failure given.
BD FocalPoint™ Slide Profiler Processing Messages Dust or dirt on coverslip The system detected images indicative of dust and scratches on the coverslip or possibly excessive mounting medium. Ensure that the coverslip is clean and within limits. Repair and rerun. If the slide cannot be successfully rerun, it must be screened by a cytotechnologist.
