The front and rear heights
of the DP do not match.
The document processor
is not properly installed in
the main unit.
The DP hinges holding
pressure is adjusted
improperly. (The hinges
vertical motion is
unsmooth and the opened
DP cannot be held. So, the
DP is skewed horizontally.)

Check the positioning of the
document processor and
tighten the screws again.
Adjust the DP hinges holding
pressure.
Replace the DP hinges.
Measures
Perform the measures for the
same image failure when
scanning the first (front) side
through the DP.
Reattach the DPCIS.
