Filling out the ACR Accreditation Forms
Leave the forms with the customer and ensure that they understand the following:
GE Medical Systems has permission from the ACR to reprint these forms, therefore these forms
can be used with the remainder the ACR Ultrasound Accreditation Application papers to submit
to the ACR for accreditation approval.
If the ACR Ultrasound Accreditation Application Forms have changed, let us know immediately.
Stress the importance of maintaining preventive and corrective maintenance records as part of a
good Quality Assurance Program.
During the next QA Check we will need to refer to the previous one completed in order to
evaluate performance trending.

Print the forms from your laptop and sign them.
If you are filling the forms out manually, ensure that everything is complete and they are
signed.
