The 1.5 Mbit/s extension board (X16DS1) is defective.

The red fault LED on the unit is lit.
The on board self-test circuitry indicates that an essential function of the unit
has failed.
Replace the 1.5 Mbit/s extension board.
As there is no equipment protection of the 1.5 Mbit/s extension board,
E11XcEQF, and E11XcUPF are always reported simultaneously as a reminder
that a hardware failure is service affecting.
