Understanding the AJ-SAP Screen
Enter old or new NHS Number. If old entered, new used if it exists.
Enter the patient's NHS number. You can enter the NHS number in the new 10-digit
format.
NOTE
If you enter an old format NHS number and a new format NHS number
exists for the patient, the new NHS number will be displayed on line two
of the display and vice versa.
Once you have entered an NHS number and pressed Enter the patient's basic details
such as name, address, date of birth, age and their doctor's name and GP code are
displayed.
Select Output Requirements fields
These fields allow you to select the criteria for the subject access print. Enter Y in
these fields if you want the details from these screens to be included in the subject
access print. The fields, the screens the details are taken from are shown in the table
below:
