Understanding the Correct Historic MR Status Screen

The screen displays the NHS number, name, date of birth and address of the patient. It

also shows whether the patient's details are "LIVE" or "DEDUCTED" on the

database. (If both "LIVE" and "DEDUCTED" are displayed, this indicates that the

patient is currently "live" but was previously "deducted" under the same NHS

Beneath these details, the patient's nine most recent Medical Record entries are

displayed, with the most recent entry (i.e. the current entry) displayed first. For each

entry, the details displayed are as follows.

The effective date for the Medical Record status change.

The Reason Code for the Change of Medical Record status. This will be either a

Transfer In / Internal Transfer Code (TA3, T, X, PT or PX), a Deduction Code (as per

the DL screen: see Section 4, Chapter 2) or a First Acceptance Code (1, 2, 4 or 5).

The Medical Record status.

The actual date of the Medical Record status change.

Prev GP/FHSA

An indication of where the Medical Records were outstanding "from" for that entry.

An indication of where the Medical Records were outstanding "to" for that entry.

This column will contain Y if an Outstanding Medical Record entry exists within the

system corresponding to that entry.
