1-4 FCC Notification and Repair Information
If this equipment is to be connected to digital services (1.544 Mbs),
report FIC 04DU9-B and SOC 6.0N.
Report the quantities and USOC numbers of the jacks required.
For each jack, provide the sequence in which lines are to be
connected, the type lines, the FIC and REN by position when
applicable.
You must also notify your local telephone company if and when this
equipment is permanently disconnected from the line(s).
