• G – GP
• P – partnership
• PG – partnership, broken down to GP level
• H – Health Authority
• Q – Q code
• R – practice
• RG – practice, broken down to GP level.
If a print has been requested, details will be printed in the following sequence:

Details will be grouped in GP code order, and MRs will be grouped
by patient name, NHS number or date order within this. GPs will not
be grouped in their partnership.
