The patient's NHS number.
Date Of Birth
The patient's date of birth, DD/MM/YYYY format.
Date
The date the label was processed.
Current GP Details
The GP's initials, surname and GP code.
Surname & Forename(s)
The patient's name will appear on the top line. It is possible that the surname and
forename(s) may exceed the length of the label, in which case the forename(s) will be
shortened to an initial(s).
Address And Postcode
The patient's current address and postcode. The minimum acceptable address will be
house, road and town.
