Multi-Level Questions List

For multi-level questions, an answer triggers another question to get more infor-
mation. Alerts in LifeStream are based upon a patients' yes responses, after the ini-
tial question has been asked.
LifeStream makes it easy to schedule a multi-level question in the Questions Win-
dow by automatically selecting all linked questions in a multi-level question. This
prevents questions from being scheduled without selecting other linked questions
in the set.
309. Are you having any problems with urination?
If Yes: 310. Are you experiencing any burning with urination or increased frequency?
311. Are you having trouble controlling your blood sugar?
If Yes: 312. Did you skip your prescribed diabetes medication today?
313. Are you having any dizziness?
If Yes: 314. Was your most recent blood sugar lower than usual?
315. Have you almost fallen in the last day?
If Yes: 316. Are you having any difficulty walking or standing?
If Yes: 317. Are you having any dizziness or light headedness today?
If Yes: 318. Did you seek medical treatment for your dizziness or light headedness?
319. Has your doctor added, deleted, or changed any of your medications this week?
If Yes: 320. Do you feel worse since starting your new medication?
If Yes: 321. Have used any over the counter medications in addition to those prescribed for you in the
322. Have you developed a cough?
If Yes: 323. Is your cough productive?
If Yes: 324. Is your sputum, mucus or phlegm thicker today?
If Yes: 325. Is your sputum, mucus, or phlegm a different color today?
If Yes: 326. Do you feel that you may have a chest infection?
Pain and Patient Health Questionnaire (PHQ-2 or PHQ-9)
Question number 500, about physical pain; and question number 400-409, from
the Patient Health Questionnaire, should only be assigned from the LifeStream
Web interface, to a fully upgraded Genesis Touch monitor supporting this feature.
Organizations without LifeStream Web should not assign these questions
manually, since they are not compatible with LifeStream's Windows application
and responses will not be displayed.
