Return Instructions

Reporting Damaged Equipment
Any defective components should be immediately reported to Brainlab support.
Brainlab support asks you for:
• System serial number (on type plate of the Monitor Cart)
• EM serial number on EM Base Station
• Component serial number (inscribed on component, e.g., Monitor Cart or Camera Cart)
• Description of problem
Repair and Replacement
Brainlab support:
• Provides you with cost estimate for repair or replacement
• Informs you when your system is expected to be operational again (usually within 48 hours)
Removing Components
Only remove defective components if instructed by Brainlab support.

Protect the component from further damage by wrapping it and safely packaging it.
Complete and return the form that was faxed to you or that accompanied replacement
part.
Securely tape the box shut.
Ship the defective component to relevant return address or follow instructions given by
Brainlab support.
