Check the media path for contaminates. Make sure that the media
path is free of excess media dust and foreign objects such as paper
clips and staples.

Check the sensor (ADF 2nd scan) for proper operation.

Enter the Diagnostics menu.

Select SCANNER TESTS > Sensor Tests.

Observe the line, "sensor (ADF 2nd scan)."
Does the display on the operator panel change every time the
sensing area of the sensor is interrupted or blocked?

Replace the connection, if necessary.
