13 12 11 10 9 8 7 6 5 4 3 2 1, where bits 1–4 = Last fault, bits 5–8
= 2nd last fault, bits 9–12 = 3rd last fault, and bits 13–16 = 4th last
fault. These four bits indicate a hex value that corresponds to the
following faults:
