 Installed options (i.e., interface and host type if applicable to the problem)
 Configuration printout (see "Printing A Configuration" in the Administrator's Manual)
 Is the problem with a new install or an existing printer?
 Description of the problem (be specific)
 Good and bad samples that clearly show the problem (faxing or emailing of these
samples may be required)
