Press [1] for the Online Center. Follow the phone tree instructions to select X–Ray
modality. When prompted, select the option for obtaining an IP address.
INSFORM.xls or INSFORM.txt for obtaining an IP Address.
The INSITE FORM is on the formatted sheet (.xls) or text sheet (.txt) that can be
found on the Sevice CD–Rom.
