Caution
When you want to specify user defined paper, use the paper size (printable area size)
instead of the output paper size.
Standard paper other than A4 and Letter is named after the "width of the output
paper", which does not match the paper size.

Vertical size when the paper feed direction is "portrait"
Horizontal size when the paper feed direction is "portrait"
Paper (roll paper) size which is actually used for printing
Printable area size specified on the printer driver
Paper sizes that are automatically registered on installation
Custom sizes that can be newly registered by users
