Subject:
Critical Accident Reporting (CAR) Procedure
Service Communication: 180
On rare occasions, an extraordinary incident which merits additional follow-up by Konica Minolta may occur.
These incidents include those where a machine apparently exhibits smoke or flame, or where personal injury
or property damage occurs. Konica Minolta must follow up on incidents of this nature on a timely basis, even if
caused by human error, to identify specific causes of machine failure and to develop solutions or corrections to
prevent future occurrences.
In an instance of this nature, notify the SSD Contact Support Center (CSC) at
you learn about the situation. The CSC will provide guidance regarding proper handling and resolution of the
situation. The CSC responsibility includes obtaining information from the field and communicating with Konica
Minolta management, branch and dealer personnel, and end users, as appropriate.
SSD will assess the incident and advise the technical manager of any actions that may be required, such as
obtaining the service history, impounding the machine, or providing specific parts for analysis.
Incidents that qualify as Critical Accidents include the following, which is only a partial list:
Bodily injury (burns, electrical shock, health
concerns)
Property damage
Smoke, flame, scorching
A complete description of situations that are covered under the CAR procedure can be found on pages 2-19
through 2-22 in the SSD Policies and Procedures, which can be found on One Stop Product Support on
MyKonicaMinolta.com using this link:
