Describe in detail any deviation from the standard IQ/OQ. The deviation must be approved by
Template-Company representative. If there is no deviation, no signature or date entry is required by
Template-Company representative (enter NA for signature and then date). If there are no deviations,
the PerkinElmer representative should enter "None" below and then sign and date.
