This disinfection certificate must be completed before the instrument is returned
to the distributor for servicing.
The certificate must be attached to the top of the outer package in which the
instrument is returned. It must be visible from the outside of the shipping
container!
I declare that the instrument in this package has been decontaminated or
disinfected to remove or inactivate any biological material that could be
dangerous to the service personnel or that it has never been exposed to any
hazardous biological material.
