CSC Premier Service Agreement

The Premier Service Agreement includes all parts, labor and shipping to and from our depot repair location
as well as your choice of a loaner instrument within 1 business day or on-site service visit by a factory-
trained service technician within 2 business days. Additionally, it includes one annual preventive mainte-
nance visit per year, which can be performed by returning the instrument to an authorized service center or
by an on-site visit by a service technician. Additional Preventive Maintenance visits are available separately.

CSC Preventive Maintenance

In order to keep the system operating at peak performance, Promega recommends that Maxwell
instruments receive a Preventive Maintenance check after 12 months of use. During this procedure, our
qualifi ed service personnel test the instrument, check parts for wear and replace them as needed.
Additionally, the system is aligned and performance is verifi ed. Documentation for your fi les is provided.

CSC Installation Qualifi cation and Operational Qualifi cation

The Installation Qualifi cation service product includes a series of formal instrument checks, delivers written
documentation of instrument functionality, and demonstrates that everything ordered with the instrument
is supplied and installed at the customer's laboratory. The Installation Qualifi cation service product involves
a site visit to:

Installation by qualifi ed Promega personnel
•
Inspection of shipping containers, instrument, and accessories
•
Comparison of items received vs. items on PO
•
Inspection of laboratory conditions
•
Review of all hazards and precautions with users
•
Confi rmation/installation of correct fi rmware version
•
Recording and documenting installation and actions
The Operational Qualifi cation service product demonstrates that the instrument functions according to its
operational specifi cations. The service product involves a site visit to perform:
•
Run operational verifi cation tests
•
Document test results
•
Train customer(s) to operate the instrument
•
Train customer(s) to use the log book
•
Complete customer specifi c log book, instrument sticker and OQ documentation
