This field determines whether the phone book record

contains information about the split between the first name
and the last name.

A zero based index pointing to the first character in the

second subfield.

If the <first_last_enabled> field is 255, then the value in this
field is ignored. The second sub-field is defined as all
characters following the first sub-field.
