Click the Cases icon:

The Case Detail page will appear:
At a minimum, select a Case Reason, Brand, and Severity from the available drop-down

menus, and provide a brief explanation of the issue in the Summary field; these fields are
required.
In addition, type the Part Number and Serial Number into the appropriate fields, and select

the Product Category and Product Sub-Category that match your Product Name / Model
Number.
