completion of commissioning. All starred (*) sections must be completed to qualify for warranty. Record any
non-conformances either in the comments section on page 2 or use a separate sheet of paper. E-mail the
completed form to Ampgard MV Group in Asheville NC along with pictures of the motor, load, and gear. Also
e-mail the as-found and as-left .eXML C445 parameter export.
