SING THE AILBACK
The Data Card slots into the mailback envelope to keep it safe while in transit or
storage.
Figure 8 shows the inside of the mailback envelope.
Dear patient, please follow the directions as marked below.

Use the mailback envelope for transporting and storing the Data Card. Place the
mailback envelope containing the Data Card in the supplied bubble mailer when
mailing the card. You can write your name and address details on the front of the
bubble mailer to assist the patient in mailing it back to you. Use a postal envelope to
mail the bubble mailer and its contents to the patient.
