Sample condensed in the loop or transfer line.

Sample did not create headspace.
Incorrect vial sampled.
Vial not punctured (may be due to mechanical failure or vial elevator jam).
Transfer line plugged.
Check that the proper vial is sampled and that the vial contains sample.
Use adapters with 10 ml vials.
