patient to predict the risk of fracture
I.
More than 3 drinks a day
If you drink more than three drinks a day, please check
the box.
The amount of one cup varies slightly from country to
country, but corresponds to 8-10 grams of alcohol. This
is equivalent to a standard size beer (285ml), a cup of
distilled liquor (30ml), a medium-sized wine (120ml),
or a pre-drinking glass (60ml).
II. Currently smoking
Please check the box if you are currently smoking.
III. Previous fracture history
To be accurate, a previous fracture refers to a fracture
resulting from a mild trauma that cannot cause a
fracture for a healthy person, or to a fracture that
occurs after adulthood.
IV. Parent Hip Fracture
Check the box if there is a history of mother's or
father's hip fracture.
V. Steroid agent
Check the box if you had taken or are now taking oral
glucocorticoids or prednisolone 5 mg or more per day
(or other glucocorticoids of equivalent volume) for 3
months or more.
VI. Rheumatoid arthritis
Check the box if you have been diagnosed with
rheumatoid arthritis
VII. Secondary osteoporosis
Check the box if you have a disease that is strongly
associated with osteoporosis.
These include: Type 1 (insulin dependent) diabetes
