For your convenience, this appendix contains a form to help you gather the information necessary to help us solve
technical problems you might have as well as a form you can use to comment on the product documentation. Filling
out a copy of the Technical Support Form before contacting National Instruments helps us help you better and
faster.

Because it helps us solve your problems more easily, we encourage you to fax us a completed copy of the Technical
Support Form and the configuration forms from the user manuals of the National Instruments hardware or software
you are using, even if you wish to contact us later by telephone. You can contact us by fax at any time at the
