The following pages contain several forms that may be useful for the site. We recommend that
the Equipment Location and Site Closeout Checklist forms be used for most sites. These forms
may be copied.

Keep this, along with a wiring map of the site, on-site with this
manual.
The dealer should also retain a copy in their customer files.
This form is a tool to protect both the end user and the dealer.
The dealer and an authorized representative of the site should
walk the property while reviewing the checklist to verify that all
items are installed correctly and working properly. Both parties
should sign off on the checklist and keep a copy.
This list may not be comprehensive to all of the work that the
installer may have performed on the site, therefore, be sure to
check all work.
