Possible Cause
Loose power and/or data cable.
SCSI bus too long.
SCSI drive not set as drive 0.
SCSI termination is done
incorrectly.

Action
Secure all signal and power cable connections.
First ensure that you are using qualified SCSI
cable. Then alter SCSI bus to correct length in
compliance with the 1.5-meter limitation for
combined cable and signals on the circuit
board.
If your SCSI bus is longer than 1.5 meters, use
shorter cables where possible, remove some
devices, or slow down the bus. See the SCSI
setup screen in AlphaBIOS or SRM.
For Windows NT users: Reset SCSI drive as
drive 0.
Terminate the extreme end of the bus.
Connect one end of the cable to the SCSI
controller and the other end to a terminator.
Make sure there is no extra termination, for
example, disable termination in drives.
Check that there is active termination only.
