Modify an entry from the DHCP forward list.

The forward entry name.
The DHCP server IP address.
The name of the relay interface, 'None' to
indicate no interface is specified.
The giaddr field to be used in relayed DHCP
packets.
Script to be run when the forward entry is hit.
