Using the Sheets
WARNING:
• Prevent possible strangulation or entanglement.
Never use playard sheet unless it fits securely on
playard mattress.
• For use ONLY with 4moms playard sheet model
number 2000795, 2000743, and 2000744. NOT
compatible with playard sheet model numbers
4M-009-14-001301, 4M-009-14-001501, OR 4M-
009-14-002001. NOT for use with 4moms breeze
bassinet mattress.
Using the Accessory
The accessory must be fully assembled and installed, including bassinet
mattress, before using. Ensure that all clips are engaged before use.
DO NOT store bassinet in the playard while playard is in use.
To help prevent heat build-up inside the playard and to avoid overheating
your child, when using bassinet DO NOT use an outdoor canopy.
WARNING:
• Never leave accessory in place when child is in
playard.

HAZARD:
INFANTS HAVE
SUFFOCATED:
• In gaps between an extra pad and side of the
bassinet
• On soft bedding
• NEVER add a mattress, pillow, comforter, or
padding. Use ONLY mattress and sheets provided
by 4moms or one specifically designed to fit the
®
dimensions of the bassinet.
FALL HAZARD:
To help prevent falls, do not use this product when the
infant begins to push up on hands and knees or has
reached 18 lbs (8 kg), whichever comes first.
