Fixes to ETSI BRI ISDN since Release 1.39H

The following changes have been made to the ISDN software:
A minor change to the European ETSI BRI code resolves an
interoperability problem with some versions of Alcatel
switches in Southern Europe. these switches return a connect
message containing a non-ETSI implementation of
"charging element", resulting in a call disconnect shown as
"DISC 101" in the trace window.
Note:
This fix is contained in the BRI ETSI code (etsi.bin) for
the Nautica CLAM and Marlin, but is linked into the Nautica
200 router image (n200eu0.bay).
A potential problem has been fixed where an actuation signal
on the ISDN line could be missed before interupts were
enabled.
