INSTRUCTION MANUAL
WARNING: READ ALL INFORMATION CONTAINED IN THE TWO INSTRUCTIONS: GENERAL
AND SPECIFIC.

SPECIFIC INSTRUCTIONS

WARNINGS: medical condition could affect the safety of the equipment user in normal and emergen-
cy use. The equipment shall be used by a person trained and competent in its safe use. A rescue
plan shall be in place to deal with any emergencies that arise during the work. Marking, alterations
or additions to the equipment without the manufacturer´s prior written consent, and that any repair
shall only be carried out in accordance with manufacturer´s procedures. The equipment shall not be
used outside its limitations, or for any purpose other than that for which it is intended. The equipment
should be a personal issue item, where this is applicable. Any dangers that may arise by the use of
combinations of items of equipment in which the safe function of any one item is affected by or
interferes with the safe function of another. Stating that it is essential for safety that equipment is
withdrawn from use immediately should: any doubt arise about its condition for safe use or it have
been used to arrest fall and not use again until confirmed in writing by a competent person that it is
acceptable to do so. It´s essential for safety to verify the free space required beneath the user at the
workspace before each occasion of use, so that, in the case of a fall, there will be no collision with
the ground or other obstacle in the fall path. The hazards that may affect the performance of the
equipment and corresponding safety precautions that have to be observed (looping of the lanyards,
chemical reagents, cutting, climatic exposure, etc). The anchor device is used as a part of a fall
arrest system, the user shall be equipped with a means of limiting the maximum dynamic forces
exrted on the user during the arrest of a fall to a maximum of 6 kN. Warning: it´s recommended the
anchor device is marked with the date of the next or last inspection. The anchor device should only
be used for personal fall arrest and not for liffiting equipment. Warning: the periodic examinations are
only to be conducted by a competent person por periodic examination and strictly in accordance with
the manufactur´s periodic examiantion procdures. Warning: it´s essential for the safety of the user
that if the product is re-sold outside the original country of destination the reseller shall provide
instruction for use, for maintenance, for periodic examination and for repair in the language of the

FS860- DUAL SLIDING BEAM ANCHOR - APPLICATION
Purpose: The dual sliding beam anchor is used as an anchorage connector for a personal fall
arrest system. It´s designed to be attached on the horizontal I-beam. The beam achor might be
used as an end termination for either a shock-absorbing or self-rectracting lifeline for fall arrest,
or with a positioning lanyard for fall restraint. Limitations: may only be installed on beams with
flanges within the adjustment range of the model (see the SPECIFICATION). Capacity: is
designed for one person use with a combined weight (clothing, tools, etc...) of no more than 140
kg. No more than one personal protective system may be connected to this equipment at one
time. Free fall: personal fall arrest system used with this equipement must be rigged to limit the
free fall to a maximum of 1.8 m. The maximum free fall must always be within the manufacture´s
free fall capacity of the system components used to arrest the fall. When a free fall greater than
1.8 m and up to a maximum of 3.6 m is possible, FALL SAFE® recommends using a personal fall
arrest system incorporating with an energy absorbing lanyard. Swing falls: before installing or
using, make consideration for eliminating or minimizing all swing fall hazards. Swing fall occur
when the anchor is not directly above the location where a fall occurs. The user must always work
as close to be in line with the anchor point as possible. Swings fall significantly increase the
possibility of serius injuris or death in the event of a fall. Fall clearence: there must be suficient
clearence below the anchorage connector to arrest a fall before the user strikes the ground or
other obstruction. The clearence required is dependent on the following safety factors; elevation
of fixed beam anchor, lenght os connecting subsystem, deceleration distance, movement of
harness attachment element, worker height and free fall distance. Distance clearance (DC) =
length of the lanyard (LL) + deceleration distance (DD) + height of suspended worker (HH) +
safety distance (SD). ATTENTION: the fixed beam anchor is designed for use with FALL SAFE®
approved components or CE certified components. Use of this equipment with non-approved
components may result in incompability between equipments and could affect the reliability,safe-
ty of the complete system. A full body harness must be worn by the user when connected to the
fixed beam anchor. When making connections with the beam anchor, eliminate all possibility of
roll-out. Roll-out occurs when interference between a hook and the attachment point causes the
hook gate to unintentionally open and release. All connectors gate must be self-closing and
self-locking. Inspection frequency: before each use inspect the beam anchor according to
following steps and check the beam anchor components for part identification. The beam anchor
must be formally inspected by a competent person other than the user annually. Record the
results in the "EQUIPMENT RECORD".
FS861 - FIXED BEAM ANCHOR - APPLICATION
Purpose: The fixed beam anchor is used as an anchorage connector for a personal fall arrest
system. It´s designed to be attached on the horizontal or vertical I-beam. The fixed beam achor
might be used as an end termination for either a shock-absorbing or self-rectracting lifeline for fall
arrest, or with a positioning lanyard for fall restraint. Limitations: may only be installed on beams

FS860, FS861 AND FS874 - INSPECTION STEPS
1 - Look for cracks, dents or deformities. Look for bening or wear on the hexagonal rod, beam
clamps, quick release lock pin and tightening handle. Ensure no parts are missing;
2 - Inspect entire unit for excessive corrosion;
3 - Ensure the quick release lock pin can be inserted through the hole on safety lock button and
locks in place;
4 - Record the inspection date and the results in the "EQUIPMENT RECORD".
ATTENTION: if inspection reveals an unsafe or defective condition remove unit from service and
destroy or return to FALL SAFE® for checj the possibility repair. WARNING: Only FALL SAFE®
or authorized are qualified to repair this equipment. ATTETION: it is the responsability of the
users to assure they are familiar with the instructions and are trained in the correct care and use
of this equipment. Users must also be aware of the operating characteristics, application limitis
and the consequences of improper use.
FS860, FS861 AND FS874 - MANTEINANCE, SERVICE AND STORAGE
Periodically clean the beam anchor by water and a mild soap solution. DO NOT USE ACIDS or
other caustic chemicals that could damage the system components. A lubricant may be applied
to the quick safety lock button and the release lock pin. Storage the equipment in a cool, dry dark
place, chemically neutral, away from sharp edges coners, souces of heat, humidity, corrosive
substances or other damaging conditions.

DS1- Remove the quick release lock pins. Then press the safety lock to adjust the sliding clamp;
DS2 - Place the Dual Sliding Beam Anchor onto beam flange on the bottom or top position of the
I-beam; DS3 - Place a sliding clamp against one side of the beam flange. Slide the other sliding
clamp against opposite side of the beam flange
