Plus Service Plan to receive your benefits. Simply complete and mail the
original enrollment card (no copies), along with the dated proof of purchase for
your printer, in the self-addressed stamped envelope.
We recommend you also complete this section and maintain this document
along with the return acknowledgment for your records.
