Maintenance and Care:

The MAYFIELD Spine Table Adaptor should be stored as a complete assembly.
If unit is dropped or mishandled, it should be inspected for damage.
(REF Inspection section of this instruction manual) If damage occurs, do not use;
return the complete device immediately to Integra LifeSciences for inspection.

Inspection of Components

A routine inspection of the components of the MAYFIELD Spine Table Adaptor should be made
before each and every procedure to assist in keeping it in good functional condition and to avoid
problems the day of surgery. This check should include the following:
1. Check to see that all the components of the base unit are available for assembly. Use the
Inspection section figures 1 or 2 of this Instruction Manual for a complete list of components.
ALL components must be available and ready for assembly or the base unit should not be used.
2. Check the adjustment of the base locking handle by following these steps.
a. Attach the adaptor to spine table and lock to the parallel rails.
b. Raise the swivel adaptor to be even with the top of the table rails, locking the handle.
c. With your hands on the swivel adaptor, push down on the adaptor attempting to move it. If
with minimal force, the adaptor moves, adjustment to increase the
locking power of the Locking Handle should be made as outlined in this manual.
d. If the handle is unusually difficult to close, adjustment to decrease the locking power of the
Locking Handle can be made as outlined in this manual. Make certain that the handle
operates properly after adjustment
3. With the Locking Handle unlocked and all the components attached but loose, raise and lower
them, rotating all joints. All should freely rotate without any "ratcheting" noise (starburst teeth
hitting together as rotated). If this ratcheting noise is heard, loosen the joint that is causing the
noise.
