• This rebate is not valid on leased, returned, refurbished or used products, or products purchased through
Internet auctions or any other unauthorized channels.
• Invoice(s) showing the purchase of an eligible MAX conference phone and Certificate of Destruction must
accompany the Claim Form.
• All Polycom SoundStation™ conference phones are eligible for this rebate. Polycom SoundStation™
accessories, such as the extended microphones, and the VoiceStation® are not included.
• Rebate valid on eligible MAX purchases made after August 15, 2007.
• Invoice showing the purchase of an eligible MAX conference phone and Certificate of Destruction must
accompany the Claim Form.
• Each Polycom unit is good for one $50 rebate toward any one MAX phone, limit one trade in per
MAX phone.
