8.6 Patient Survey
Depending on the Patient Zone a survey may be completed to document the patient's
demographics, pain history, and experience with SCS.
Note: The questions in the Survey may vary between Patient Zones.
1. Tap the Navigation Menu icon
2. The patient must give consent to collect the survey information and for information
to be backed-up remotely on Saluda Medical servers.
3. Read the consent language to the patient and record the answer on the survey page
(Figure 8.3).
Note: The Patient ID on the CLS programmer page will be in green text if the consent
answer is Yes and orange if the consent answer is No.
4. The Session List displays all the surveys that have been created (Figure 8.4).

Start Survey
1. Tap on the "+ New Session" button to create a new survey (Figure 8.4).
a. For a new patient a Setup survey will be created.
b. For an existing patient a Follow Up survey will be created.
2. Tap on the open icon
unsaved survey.
3. Complete the survey questions and then tap the "Save and Close" button to save the
survey data to the stimulator. Otherwise tap the "Back to Sessions" button to exit
the survey without saving.
4. Tap on the open icon
