INDUSTRY CANADA NOTICE

This Class B digital apparatus complies with
Canada ICES003.
Notice: The Industry Canada label identifies certified equipment. This
certification means that the equipment meets certain telecommunications network
protective, operational and safety requirements. The Department does not
guarantee the equipment will operate to the users satisfaction.
Before installing this equipment, users should ensure that it is permissible to be
connected to the facilities of the local telecommunications company. The
equipment must also be installed using an acceptable method of connection. In
some cases, the company's inside wiring associated with a single line individual
service may be extended by means of a certified connector assembly (telephone
extension cord). The customer should be aware that compliance with the above
conditions may not prevent degradation of service in some situations.
Repairs to certified equipment should be made by an authorized Canadian
maintenance facility designated by the supplier. Any repairs or alterations made
by the user to this equipment, or equipment malfunctions, may give the
telecommunications company cause to request the user to disconnect
the equipment.
Users should ensure for their own protection that the electrical ground
connections of the power utility, telephone lines and internal metallic water pipe
system, if present, are connected together. This precaution may be particularly
important in rural areas.
Caution: Users should not attempt to make such connections themselves, but
should contact the appropriate electric inspection authority, or electrician, as
appropriate.
Notice: The Ringer Equivalence Number (REN) assigned to each terminal device
provides and indication of the maximum number of terminals allowed to be
connected to a telephone interface. The termination on an interface may consist
of any combination of devices subject only to the requirement that the sum of the
Ringer Equivalence Numbers of all devices does not exceed 5.

LIMITED WARRANTY
for Plantronics Non-commercial Products Purchased in the US and Canada
This warranty covers defects in materials and workmanship of Commercial
Products manufactured, sold or certified by Plantronics which were purchased
and used in the United States and Canada.
This warranty lasts for one year from the date of purchase of the Products.
This warranty extends to you only if you are the end user with the original
purchase receipt.
We will, at our option, repair or replace the Products that do not conform to
the warranty. We may use functionally equivalent reconditioned/refurbished/
remanufactured/pre-owned or new Products or parts.

Canada call (800) 540-8363. If you need additional information, please
contact our service centers at the numbers provided.
THIS IS PLANTRONICS' COMPLETE WARRANTY FOR THE PRODUCTS.
This warranty gives you specific legal rights, and you may also have other
rights which vary from state to state or province to province. Please contact
your dealer or our service center for the full details of our limited warranty,
including items not covered by this limited warranty.
