Riving knife not in line with
blade
Saw blade out of alignment
Warped workpiece
Feed rate too fast
Carriage assembly loose

Miter not locked firmly
Bevel not locked firmly
Rip lock not locked firmly

What to Do
Align riving knife to blade
Re-align
