Intended Use of the Product
The Dyna-Form Overlay Superior is a pressure redistributing foam mattress intended to be used in the prevention or treatment of pressure ulcers.
The mattresses is suitable for persons who are at high risk of pressure ulcers, including those with category 1 or category 2 pressure ulcers,
alongside an individualised care plan.
Indications for Use
The Dyna-Form Overlay Superior is intended to be used by all kinds of users, including lay persons, in all healthcare environments including
professional healthcare facilities and home healthcare.
The Dyna-Form Overlay Superior is an overlay mattress, and therefore should be used in combination with a secondary lower mattress. The
Dyna-Form Overlay Superior should not be used as a pressure redistributing mattress on its own, without a secondary mattress below.
Contraindications for Use
There are no known contraindications for use.

Warnings and Cautions
Warnings
• Which mattress is suitable for your individual use should be decided by your clinician who will also determine the most appropriate pressure
ulcer prevention or treatment plan and the relevant nursing procedures.
• Always ensure that the mattress is the correct size for the required bed.
• The usage of additional waterproof mattress covers, electric blankets, sheepskins, or other similar items which may reduce the effectiveness of
the pressure redistributing properties of the mattress or lead to an additional increase in body temperature such as to cause skin maceration is
not advisable.
• As this is an overlay product, ensure that the mattress is securely fastened to the bottom mattress, using the elasticated straps provided,
before a user lies on the surface.
• Ensure that no sharp objects damage the mattress or mattress cover.
• The printed side of the mattress cover must always be facing upwards. Head/foot alignment is indicated on the cover.
• Always ensure that the distance between the top surface of the Dyna-Form Overlay Superior and the top of the bed side rails is ≥20mm.
• Inspect the inner and outer surfaces of covers, seams and zips regularly for signs of damage.
• If the cover is stained, soiled, or torn, the foam core should also be examined.
Cautions
• It is recommended that an audit of all mattresses is carried out at least annually.
• Mattresses can become damaged if stored incorrectly. It is advisable that a mattress is stored in a clean and dry environment. It is not advised
that mattresses are stored near heating devices, such as electrical fires or radiators.
• Take care when handling the mattress to ensure no damage to the cover or foam.
• Avoid contact with abrasive surfaces, and do not drag the mattress.
