ER CAST post for one-piece cast
1. Indication
- prosthetic coronal restoration of partly or completely destroyed upper and lower anteriors, canines and
premolars
- for one-piece cast
2. Contraindication
- subgingival caries
- circular isogingival destruction
- extremely small teeth or roots
- in case of cervically funnel-shaped open lumen when no post adaptation to the canal wall is possible
- full circular crown seat on natural sound hard substance is not possible
3. Clinical sequence/proper use
3.1 Preparatory, general procedure
3.1.1 Tooth/root has been endodontically treated and prepared according to the indication
- endodontic treatment
- sealing the apical third
- with partly destroyed teeth prepare the cavity for example with diamond instrument 835KR or 801.
- with severely destroyed teeth prepare the core surface until only healthy tooth substance remains and give it a
concave shape using ball-shaped diamond instrument 801 (Fig. 1)
- Reduction of the clinical crown to app. 2 mm above the cemento-enamel junction, proximally
3.1.2 Select the matching CAST positioning post
- use the radiograph and measuring sheet 9751 to identify a suitable post diameter and post length according to
the anatomy of the tooth
- post should extend to a position of app. 2 – 4 mm short of the apex
3.1.3 Removal of the root filling to app. 2 – 4 mm short of the apex with pilot drill 183LB corresponding with the
selected post size 050, 070, 090 or 110
- mark the required preparation length on the pilot bur using a stop (Fig. 2)
- engine speed: 1000 – 5000 rpm
3.1.4 Enlarge root canal according to the selected CAST positioning post
- with the respective reamer 196; if clinical crown is still available use reamer 196L (Fig. 3).
- mark the required preparation length on the reamer with depth gauge 74L12
- Engine speed: 1000 – 2000 rpm
- reaming should be carried out in an intermittent way with minor pressure until required depth is reached. In the
meantime the root canal should be rinsed and the reamer should be cleaned with brush 9791
- manual reaming is recommended
3.1.5 Establishing the central inlay cavity (maximum depth 1 – 2 mm)
- for antirotational enhancement and stabi lity of the buildup prepare a longitudinal oval tapered box cavity
(Fig. 4)
- if root cross-section is large enough use root facer 120D.204.030
- engine speed: 2000 rpm
- or cylinder diamond instruments, e.g. 835KR
3.1.6 Check post seat by inserting the selected post
3.2 Direct modellation of the buildup
3.2.1 Taking impression of the auxiliary cavity
- insulate auxiliary cavity with silicone or vaseline oil.
- apply burn-out modelling acrylics (e.g. Palavit G) to the coronal area of the post circularly
- insert post
- fill auxiliary cavity with modelling acrylics (Fig. 5)
- Remove and reinsert post for control
3.2.2 If necessary, shorten root post coronally
3.2.3 Build up core (Fig. 6)
- with burn-out modelling acrylics
- either freehand or with auxiliary devices (e.g. Frasaco / Strip crown)
- allow acrylics to cure
- Initial preparation of the core
3.2.4 Remove modelled post buildup for further processing in the laboratory and provide root with a temporary
post during the laboratory work
3.3. Temporary treatment
3.3.1 Temporary treatment during laboratory work with ER TMP posts P75L14, P422L14, P76L14, P77L14
Note: The ER TMP post has been designed in particular for temporary treatment and is not suited for permanent
buildups.
Note: The post tip of the ER TMP post is 2 mm shorter than the CAST post tip. If the post is inserted in the root
canal a hollow space of 2 mm will remain apically. This hollow space offers the advantage that the tooth can be
provided with a medicated inlay if an inflammation needs to be cured.
3.3.2 Apply ER TMP post with temporary cement
Note: If the patient wears the temporary appliance only for a short time, no fixation agent is necessary. The

3.3.3 Remove excessive cement
3.3.4 Fabrication and cementation of temporary crown
3.4 CAST post buildup in the laboratoryTechnical data of CAST positioning posts:
- burn-out acrylics leaving no residues, polymethylmethacrylate (PMMA)
3.4.1 CAST the buildup made by the dentist
3.4.2 Remove cast from the embedding compound (Fig. 7)
3.5 Insert cast (post buildup)Post buildup fabricated and supplied by the laboratory
3.5.1 Remove temporary restoration (temporary crown with ER TMP post)
3.5.2 Clean root canal and auxiliary cavity
Note: If necessary, remove cement residues from the canal with reamer 196 (or 196L), preferably manually
- rinse root canal (e.g. with H
3.5.3 Seat control
- insert cast post buildup in the root and check seat
- eliminate irregularities, if any, with suited instruments.
3.5.4 Mechanical conditioning of the canal wall with roughening instrument 196D
- turn instrument manually in the root canal without pressure (3 – 5 turns) (Fig. 8)
3.5.5 Rinse root canal and dry.
- rinse root canal (e.g. with H
3.5.6 Cementation of completed post buildup (Fig. 9)
- Mix suited fixation agent to a creamy consis tency (e.g. zinc phosphate cement, glass ionomer cement)
- coat post shank with fixation agent
- insert post buildup slowly into the canal and auxiliary cavity with moderate pressure so that excessive cement
can escape and fix by applying light pressure with your thumb.
- after curing remove excessive fixation agent
4. Further treatment
4.1 For the definite preparation the following rules have to be observed:
- final preparation of the crown core according to the intended crown (Fig. 10). Make sure that the core is
circularly surround ed by natural caries-free tooth substance
- the crown has to surround the root now reinforced with a post in a parallel zone which is at least 2 mm wide so
that forces exerted are distributed equally on the entire tooth
4.2 Impression and temporary restoration in the usual manner
4.3 Fabricate and insert crown
5. Recommendations for maintenance
All system components are supplied in non-sterile condition. Attention: the CAST posts cannot be sterilized.
Therefore, they must be cleaned and disinfected with medical alcohol. The instruments have to be disinfected,
cleaned and sterilized prior to each use. The instruments are to be disinfected with anticorrosive disinfecting and
cleaning agents for rotary instruments. Regarding recommendations for use (immersion time, concentration,
suitability) of the disinfecting and cleaning agents see manufacturers' instructions. Remove all residues of
disinfecting and cleaning agent with water and allow to dry (e.g. by air-stream). Do not store instruments for a
longer period in wet or humid condition. Make sure that they do not come in contact with each other during
ultrasonic cleaning! Inspect cleaned instruments visually. Damaged and dull instruments must not be used any
longer. Sterilization of the instruments is carried out using the known methods in the autoclave according to the
specific provisions on the sterilization of medical products in your country. The instructions of the respective
device manufacturer have to be observed. After sterilization, check instruments for defects on the surface.
Corroded instruments must not be used any longer. The operator of medical products is responsible for seeing
that proper treatment is carried out by qualified personnel using the appropriate materials and suited equipment.
