1. System overview
see table 1
2. Indication
- prosthetic coronal restoration of destroyedupper and lower anteriors, canines and premolars
- basic requirements:
- adequate adaptation of the post to the canal wall along its entire length
- the lower retention element of the retention head can be lowered into the root crosssection
- successfully completed root canal treatment
3. Contraindication
- in case of cervically funnel-shaped canal lumen, where an adaptation of the post to the canal wall along its
entire length cannot be achieved
- the lower retention element of the retention head cannot be lowered into the root cross section
- subgingival caries
- circular isogingival destruction
- a full circular embrace of the core by natural, caries-free hard dental substance cannot be provided
4. Clinical sequence/proper use
4.1 Preparatory, general procedure
4.1.1 The tooth/the root has been endodontically treated and prepared according to indication
- endodontic treatment
- sealing the apical third
- in case of deeply destroyed teeth prepare the core surface until only sound substance is left. Give core a
concave shape using the ball- shaped instrument 801 (fig.1)
- reduction of the clinical crown to approx. 2 mm above the cemento-enamel junction, proximally
4.1.2 Select post to match the tooth to be restored
- in consideration of the clinical conditions
- choose the post diameter on the basis of the radiograph and the measuring sheet 9751, and the post length
according to the anatomic features of the tooth
- post should extend to approx. 3 - 5 mm short of the apex
- to ensure stability, the post must be anchored as closely to the root wall and as deeply into the root canal as
possible (at least 2⁄3 of the length of the root)
4.1.3 Removal of the root filling to approx. 3 – 5 mm of the apex with pilot drill 183LB of the chosen post size
050, 070, 090 or 110
- mark the required preparation length on the pilot bur using stop 9114 (fig. 2)
- engine speed: 1.000 – 5.000 rpm
4.1.4 Prepare central inlay cavity (maximum depth 1 - 2 mm)
- for antirotational protection and stabilisation of the build-up prepare a longitudinal/oval tapered auxiliary cavity

- by slightly tilting the root facer
- with root facer 120D.204.030
- engine speed: 2.000 rpm
- prepare inlay cavity with an antirotationalgroove (fig. 4)
- with cylindrical diamond abrasive instruments, for example 835KR
4.1.5 Enlarge root canal according to the selected post size
- using the corresponding reamer 196 (fig. 5)
- mount a depth gauge matching the root post 45L9, 45L12 or 45L15 on to the reamer
- engine speed:1.000 - 2.000 rpm
Reaming should be carried out in an intermittent way with minor pressure until the required depth is reached. In
between the root canal should be rinsed and the reamer should be cleaned with brush 9791.
- enlarge preferably by hand
- clean and dry root canal
4.1.6 Check proper fit of the prepared canal
- by inserting the selected post
- adjust possible inaccuracies with reamer 196
4.1.7 mechanical conditioning of the canal wall with roughening instrument 196D
- turn instrument manually in the root canal(3 - 5 turns), without applying any pressure (fig.6)

4.1.8 Rinse and dry root canal and inlay cavity
- rinse root canal (e.g. H

- degrease in medical alcohol
4.2 Cement root post
- mix suitable fixation agent to a creamy con sistency (e.g. zinc phosphate cement, glass ionomer cement or auto
polymerising bonding composite)
- coat post shank with fixation agent
- insert post slowly into the canal or into the auxiliary cavity, exerting only moderatepressure to enable excess
cement to escape. Fix by pressing thumb on to the cavity.
4.2.1 After curing remove excess fixing agent
5. Further treatment
5.1 Build up core with composite or glass ionomer cement (fig. 7)
- make sure that sufficient material is inserted into the inlay cavity and the gaps of the retention head
5.1.1 Prepare core according to the intended design of the crown
- ensuring that the core is circularly surrounded by natural caries-free tooth substance
- in case of full ceramic crowns use shoulder preparation
- in case of metal ceramic crowns keep a preparation margin of 2 – 3 mm (fig. 8)
- the purpose of the post is to stabilise the root. The crown has to surround the post reinforced root in an at
least 2 mm wide parallel zone so that forces exerted are distributed on to the entire tooth

6. Recommendations for maintenance
All system components are supplied in non-sterile condition. Therefore posts and instruments have to be
disinfected, cleaned and sterilized prior to each use. The instruments are to be disinfected with anticorrosive
disinfecting and cleaning agents for rotary instruments. Regarding recommendations for use (immersion time,
concentration, suitability) of the disinfecting and cleaning agents see manufacturers' instructions.Remove all
residues of disinfecting and cleaning agent with water and allow to dry (e.g. by air stream). Do not store
instruments for a longer period in wet or humid condition. Make sure that they do not come in contact with each
other during ultrasonic cleaning! Inspect cleaned instruments visually. Damaged and dull instruments must not
be used any longer. Sterilization of the posts and instruments is carried out using the known methods in the
autoclave according to the specific provisions on the sterilization of medical products in your country. The
instructions of the respective device manufacturer have to be observed. After sterilization, check instruments for
defects on the surface. Corroded instruments must not be used any longer. The operator of medical products is
responsible for seeing that proper treatment is carried out by qualified personnel using the appropriate materials
and suited equipment.
